SAFETY IN AESTHETIC: ULTRASOUND AND TREATMENT PLANNING
Here I talk through future treatment planning for a customer who wished to have lip filler. This shows they have anatomical variation meaning they are more at risk of blood vessel blockage from dermal filler that could lead to temporary / this blood vessel. This 2% of the population means for every 100 people 2 of them have this variation. I know I want to avoid this always. I do not like taking risk. This is why I use ultrasound in my clinic.
Aspiration means pulling back on the plunger to see if you see blood. This is what most practitioners do to safety check to ensure not in a blood vessel. However, this is not 100% accurate and can be misleading. Some reasons why are if you apply too much pressure to collapse the vessel, subsequently move or the product blocks the blood. Some may then suggest using a cannula- a blunt ended device that theoretically should push blood vessels out of the way and not pierce them. This is not 100% safe either as 25G cannulas can pierce blood vessels too. Treatment under ultrasound guidance is best advised to visualize the position accurately.
Lastly, as a practitioner, would you not want to know what you are about to take on risk wise? From a customer viewpoint, don’t you want to know your risk to reduce potential complications?
From both viewpoints, I like to be certain and prepared so I use ultrasound.
Alternative treatment suggested – treatment under ultrasound guidance or skin booster treatment which has minimal risk of serious complication unlike filler would be an option too. Not treating is always an option. Discussion with the customer here is key to fully inform and come to a sensible, safe treatment plan.
